Blackboard User Enrollment Request

This form is used to add up to five users to any single Blackboard site. It is intended for use by faculty and staff who preside over the course which is to be modifed.

When requesting that a user be added to a Blackboard course or site, they must be assigned a role. The role designates how much access they have to a given Blackboard site. The roles are explained in detail here.

( Must be a valid @uncp.edu email address. )
If this request is for an academic course please specify the department, Course ID, section number and term of the course.
For all other Blackboard sites please list the Course ID here. There is no need to list an academic course here a second time.
