Request Forms

Faculty and Staff Blackboard Forms

form iconBelow you can find forms for specific Blackboard requests. These forms are intended to be used by faculty and staff working as instructors over Blackboard sites. None of these forms are applicable to students.

Blackboard Course Merge Request

This form is used to request for multiple Blackboard courses to be combined a single Blackboard course shell. If you are teaching multiple sections of a course or have a cross listed course and you need all the students to be in the same actual couse in Blackboard, this is the form you want. 

 

 

Blackboard User Enrollment Request

This form is used to request one or multiple (up to five) users to be added to a Blackboard site. This form is often used for:

- Additional Instructors
- Tutors
- Teaching Assistants
- Student Instructors

 

Blackboard Faculty Student Access Request

This form is used to request a faux student account be added for a faculty member into their online courses.

 

Blackboard Course Archive Request

This form is used to restore a previously archived academic course from backups.

 

Blackboard Batch User Enrollment Request

This form is used to request many users to be added to a Blackboard site in mass and must be acompanied by a .XLSX, .CSV or .ODS file. This form is especially useful for people who are using blackboard for workshops or for Blackboard sites having large changes in enrollment.
