FALL 2007 Intern Checklist

 

  • September 27, 2006 – Placement Application For The Professional Semester form to be completed during session.     

 (All student candidates are to schedule an appointment with the appropriate program coordinator to discuss a requested

        placement by November 10, 2006)    (Mandatory)   

 

  • October 25, 2006- All Fall 2007 Interns are to meet with a staff member from the Career Center in the J. B. Chavis  University Center, room #213 at 10:00 a.m. for a resume workshop.  The resumes are due in our office no later than November 30, 2006

              

  • October 25, 2006 - Enrollment for Professional Semester – to be typed – front and back.  This form is to be returned to our office.

 

  • October 25, 2006 - Advisor’s Recommendation for the Professional Semester – to be typed – and all signatures procured before submitting to our office.

               

  • October 25, 2006 - Registrar’s Statement and Application Degree to be typed and signed.  This form is to be completed and submitted to the Registrar’s Office.

               

  • October 25, 2006 - Employee Security Data form if requesting placement in  Cumberland County Schools.

               

  • October 25, 2006 - Program Standards Progressive Midpoint Assessment  (CARE) to be completed all signatures procured before submitting to our office.   Our office will not process your Enrollment Application without receiving this completed document.

 

  • April 18, 2007 - Copy of current Health Insurance Card.

 

  •  March 5, 2007 - Schedule health exam after this date. 

  • April 18, 2007 – All health exams will be completed and forms submitted to our office.

 

  • April 18, 2007 - Health form – to be typed – follow physician schedule in #7 - Return this form with signatures  to our office no later than

 

  • April 18, 2007 - North Carolina Public Schools Health Examination Certificate –    Follow physician schedule in #8 – Take typed form to Student Health Services – Return the completed form to our office.

 

  • February 21, 2007– All Fall  2007 Interns will meet at 10:00 a.m. in room #222 for field experience placement assignments, letter to be presented to the prospective principal and discuss the Teacher Candidate Work Sample requirement for the pre-internship semester.    (Mandatory)

 

  • The student  intern placement and schedule for the professional semester will be mailed to your listed address  approximately two weeks prior to classes beginning.

***Students are required to be admitted into the Teacher Education Program one semester prior to the professional             semester.  (excluding summer sessions).

 

***All forms are to be submitted to our office on the due date.  A student will not be permitted to begin the professional                       semester without completing all forms in the enrollment packet.

 

***A Final CARE Review Form will be forwarded to the Office of USP by the Program Coordinator at the completion of the professional semester.

 

***An appeal form must be filed no later than midterm for the semester prior to the internship.
