of North Carolina at Pembroke must be met before a transcript request
can be processed!
Complete the Transcript
Request Form with your enrolled name, address, social security number,
dates of attendance, date of birth, and send the request to:

- Sign the request; we cannot process any transcripts without your
signature.
- Indicate in your letter where the transcript is to be mailed and stipulate
whether the transcript is undergraduate or graduate.
- If the transcript is issued directly to the student, the following inscription: "issued
to student" will be indicated on the transcript. To ensure it is accepted
as official do NOT tamper with or open the envelope.
- Include a check or money order payable to UNCP for the full amount of
the transcript. (Please note there is a $ 25.00 service charge on
all returned checks.) You may also pay by credit card (Visa, Master
Card). Please include account number and expiration date in the request
or call 910.521.6206 and pay by telephone.

- We will accept faxed requests for transcripts
if they contain the proper information and payment information
is included on the request or by telephone. Our fax number is 910.521.6328.
If you pay by phone, please indicate that on your transcript request
by including the receipt number.
- Transcripts may be faxed, but they are not official. Most universities
require an official transcript with a clear seal provided.
- A transcript can be picked up in person during regular business hours.
A picture ID is required.
