declaring a major or minor and social security change

Each student should complete a Declaration of Major or Change of Major form and submit it to the Office of the Registrar. Forms may be obtained from the Department Secretary or Office of the Registrar. If a student's name has changed or if there is an error in the social security number, he/she should complete a Name/Address Change Form in the Office of the Registrar along with a copy of proper documentation.
