Terminal Degree AwarD re-application

APPLICATION DEADLINE FOR SUMMER AND FAll TERMS: April 15, 2011

If you have received support from the terminal degree program in the past and would like to request continued funds for the upcoming semester, please complete the following forms and submit to the Project Coordinator in Lumbee Hall 336:

Form A (coversheet) (Word)

Form B (degree plan) (Word)

Form C (budget narrative) (Word)

Before reimbursements can be processed for future semesters the "Semester Summary " must be on file with the Project Coordinator.
