Please check appropriate reason for appeal (brief explanation and supporting documentation required)
The use of library material is a contractual agreement between the borrower and the Library. As a condition of using the Library, users are expected to familiarize themselves with the rules of borrowing and take responsible action to insure that material is returned or renewed in a timely manner.
Therefore, please note that appeals for the following reasons will not be considered:
Laptop fines will not be waived as these are agreed to in advance when you sign the Laptop Computer Agreement Form prior to borrowing a laptop.
Please provide your explanation, statement, or question here. Please include the amount of the charge and which items the charges are for. Use back if necessary. Attach supporting documentation.
Return form to the Circulation Desk. You will receive a reply within one week. Who reviews appeals? Appeals are reviewed first by the Access Services/Reference Librarian. If the appeal is denied, the patron may submit an appeal to the Assistant Dean for Research Services. If the second appeal is denied the patron may make a final appeal to the Dean of Library Services. The decision of the Dean is final and may not be further appealed. Patrons should note that in the majority of cases, appeals that are initially denied are unlikely to be overturned further up the appeals hierarchy.
