Below is information about the form as well as answers to frequently asked questions.
Before, your IRB protocol can be reviewed, each PI MUST complete OHRP training. PI's must forward a copy of their training certificate to the IRB Chair upon completion. All protocols received that do not have a training certificate for the PI on file will be returned effective February 1, 2009. Directions for training can be found on the Related Links page.
You should submit all forms electronically to irb@uncp.edu. In addition, please submit 1 signed copy of the forms to the IRB Chair through campus mail.
What forms should you submit?
For all studies, submit Form A, which consists of these sections:
Part A-1: Contact Information, Agreements, and Signatures
Part A-2: Summary Checklist
Part A-3: Conflict of Interest Questions and Certification
Part A-4: Questions Common to All Studies
Part A-5: The Consent Process and Consent Documentation
For studies that involve direct interaction with human subjects (any contact with subjects including questionnaires, interviews, focus groups, observation, treatment interventions, etc), submit:
For studies that use existing data, records or human biological specimens, submit:
***Note***: You should submit Parts B or C only as applicable. If the study involves both direct interaction and use of existing materials, use all parts, A, B, and C.

Who can serve as the principal investigator (PI) for a study?
The PI is the person who will personally conduct or supervise this research study. Under most circumstances, this will be a faculty member. For IRB communication purposes, a trainee/student may be listed as PI. However, a faculty advisor must be identified, who holds ultimate responsibility for ensuring that this project complies with all University, regulatory, and fiscal requirements.
