address and name change instructions

Are you changing your Address or Name and unsure   of which forms you should fill out? Well follow this guideline and you’ll be done before you know it.

If you are a Permanent Employee of UNCP follow these guidelines:

  1. The one form you will definitely need is the Address/Name Change Form (Word) . 
  2. If you are enrolled in the State Health Plan, you will need to complete the State Health Plan/PPO Change Form (PDF).
  3. If you are enrolled in NCFlex, you will need to update your information online on the NC Flexible Benefits Site.
  4. If you are enrolled in an ORP then you need to contact your representative.
  5. If you are enrolled in TSERS then you should see an HR representative or visit our website and download the forms from there.
  6. If you have 401k / 403b / 457 plans then you need to contact your representative.
  7. If you are changing your name, you need to bring your new social security card so that we can view it. You must also complete the Address/Name Change Form above.
