address and name change instructions

Are you changing your Address or Name and unsure   of which forms you should fill out? Well follow this guideline and you’ll be done before you know it.

If you are a Permanent Employee of UNCP follow these guidelines:

  1. The one form you will definitely need is the Address/Name Change Form (Word) . 
  2. Are you enrolled in the State Health Plan or one of the PPO Plans?   If so, you will need the State Health Plan/PPO Change Form (PDF).
  3. Are you enrolled in NC Flex? If so, you will need the NC Flex Change Form (PDF).
  4. If you are enrolled in ORP then you need to contact your representative.
  5. If you are enrolled in TSERS then you should see an HR representative or visit our website and download the forms from there.
  6. If you have 401k / 403b / 457 plans then you need to contact your representative.
  7. If you are changing your name you need to bring your new social security card so that we can make a copy of it for payroll purposes.
