PRE-PROFESSIONAL STUDENTS
STUDENTS IN THE PROFESSIONAL PHASE OF THE ATEP
The following forms must be completed for each clinical rotation:
- Rotation Orientation Checklist (first day of each new rotation)
- Athletic Training Student Contract (first day of each new rotation)
- Monthly Calendar
- Clinical Experience Documentation Form (each time you go to a clinical site for rotation)
- Clinical Site Evaluation (at the end of each rotation)--online form coming soon!
- ACI/CI Evaluation (at the end of each rotation)--online form coming soon!
Please submit all forms, except for the Clinical Experience Documentation Form and the calendars, online.
