PRE-PROFESSIONAL STUDENTS
STUDENTS IN THE PROFESSIONAL PHASE OF THE ATEP
The following forms must be completed for each clinical rotation:
- Rotation Orientation Checklist (first day of each new rotation)
- Athletic Training Student Contract (first day of each new rotation)
- Monthly Calendar
- Clinical Experience Documentation Form (each time you go to a clinical site for rotation)
- Clinical Site Evaluation (completed by the student at the end of each rotation)
- ACI Evaluation (completed by the student at the end of each rotation)
The following evaluation forms are specific to the semester in which the student is enrolled. Students should complete the Self-Evaluation at the mid-rotation point and at the end of each rotation prior to meeting with the ACI. The ACI's will complete the Student Evaluation at the mid-rotation point and at the end of each rotation.
- Semester 1 Student Evaluation (completed by the ACI)
- Semester 2 Student Evaluation (completed by the ACI)
- Semester 2 Student Self-Evaluation (completed by the student)
- Semester 3 Student Evaluation (completed by the ACI)
- Semester 3 Student Self-Evaluation (completed by the student)
- Semester 4 Student Evaluation (completed by the ACI)
- Semester 4 Student Self-Evaluation (completed by the student)
- Semester 5 Student Evaluation (completed by the ACI)
- Semester 5 Student Self-Evaluation (completed by the student)
Please submit all forms, except for the Clinical Experience Documentation Form and the calendars, online.
