SOCIAL HISTORY
Tim Kozlowski, a 20-year-old male, was admitted to University Hospital on1/09/00 and was transferred to Pembroke State Hospital on 10/22/00 with the diagnosis of paraplegia at the T-12 level.  The injury occurred as a result of falling out of a tree on a camp out while under the influence of alcohol. According to Mrs. Kozlowski (his mother), when Tim was at University Hospital his emotional state could be described as stable.  As soon as he was transferred to Pembroke State Hospital, he entered a state of depression and anger -- probably the result of being faced with the reality of the non recoverability of his injury.

Tim, the third oldest, of five children was born and reared in Robeson County, North Carolina.  After he graduated from high school, Tim acquired employment with Liberty Bell Home Sales where his father is also employed.  Since graduating from high school, Tim had pondered the idea of upgrading his vocational skills by pursuing the study of electronics in one of the near-by technical schools.  Tim is thinking more seriously about technical school than he did prior to the accident.  However, he seems to feel unsure about his electronic aptitude.  He has had no real experience in electronics and does not seem able to explain why he has interest in that area.  Mrs. Kozlowski pointed out that Tim lacked motivation in his studies while attending high school.  She seemed to indicate that she was not sure that Tim has the motivation necessary to complete a degree granting program of any kind.

During the initial interview, Tim expressed -- in a somewhat angry tone -- that he will walk again as a result of the miraculous intervention of God.  Tim believes the doctors when they discuss his future functioning and realizes that there is little that medical science can do to make him walk; but he still contends that he will be on his feet.  He acquired this notion from a "cure" he received when 11 years old while lying on an alter at his mother's church.  Mrs. Kozlowski brought Tim to the church because the doctors could not do anything about Tim's multiple illnesses.   Mrs. Kozlowski feels that she has given her son unrealistic expectations of his recovery.  She indicated that she feels depressed about this situation because Tim's thoughts about being cured by God have prevented him from doing anything to improve himself.

Tim has a girlfriend (Mary) with whom he is very close.  It is obvious that Mary is very concerned about Tim's recovery.  Her expectations seem much more realistic than Tim's.  She has been involved in conferences with medical staff and appears to understand the limitations Tim will have the rest of his life.  Mrs. Kozlowski seems to feel that the two (Mary and Tim) will wed and is concerned about Tim's sexual functioning and his ability to father children.  At this point, neither Tim nor Mary give the impression that they have concerns.   Mr. Kozlowski  found the two in bed together and thought that Mary might do Tim more damage (psychologically and physically) by being sexually active.

In general, Tim appears to have a very close relationship with his mother, while having a good working relationship with his dad.  Tim is also very close to his girl friend.  Tim does not seem to be receptive to discuss his relationship with his four siblings.

Tim stated two concrete goals: 1) Tim wants to learn to take care of his bladder and bowl needs.  2) Tim wants to leave Pembroke State Hospital as soon as possible.  It is evident that he does not like it here.  He expresses a great deal of anger toward the staff.

At this point, Tim has no financial resources to assist with the payment of this hospital stay.  His dad does not have insurance for Tim, and the combined income of Tim's parents is approximately $23,678.00 per year (for the parents and their three dependent children).

IMPRESSIONS
Tim is a depressed and angry individual who has acquired unrealistic aspirations of his recovery through supernatural intervention.  These aspirations are hurting him because he feels that he does not have to work toward recovery.   "God will do it all," he often states.  Mrs. Kozlowski unintentionally instigated these thoughts in Tim.   It is clear that Tim will have to develop some vocational skill, if he does not want to reply upon Welfare.   However, he seems to feel unsure about his abilities to develop new vocational skills while his mother seems to feel that he does not have the "drive" may be related to his expectations of supernatural intervention.

At this point, Tim's family relationships can be described as stable and having strong ties with his mother and girlfriend.  His mother is supportive, but this kind of support might tend to give Tim the notion that a complete recovery will occur.  Mary's relationship with Tim appears to be unclear at this point.  They appear to have a close relationship; however, this relationship may be based on pure sympathy rather than emotional commitment.  The relationship between Tim and mother, plus Tim and girlfriend have the potential for being both destructive and supportive for recovery.  It is necessary to probe both relationships in depth.

PROBLEMS

  • Financial needs to cover the cost of hospitalization and equipment (i.e., wheelchair).
  • Tim's vocational future seems "unsure."
  • Mrs. Kozlowski feels that Tim may not have the motivation to finish any kind of vocational training.
  • Unrealistic view of total recovery influenced by religious beliefs.
  • Tim does not have control over his bowl and bladder but has strong motivation for such control.
  • Tim's relationship with his girlfriend has the potential of being supportive or destructive -- depending on her agenda.
  • One important expectation that will affect both Tim and Mary is their attitude toward and their knowledge of human sexuality and spinal cord injuries.
  • Mr. Kozlowski's concern about damage that Mary may inflict on Tim.
  • The unknown effects that siblings may have upon Tim.
  • Since Tim's injury occurred while under the influence of alcohol, alcohol may become a major problem for Tim.
