Citations Used: Using APA style, list the citations used for this report.   One citation is acceptable.  If appropriately used, extra credit will be given for using additional citations.

Leading Theoretician(s):  Within no more than two paragraphs, write a biography of the person who originally constructed this theory.   If the theory has a long history in which no single person can be identified, a flowchart can be used [see Hunnex (1986) as an example].  

Purpose and Characteristics:
  Within no more than five paragraphs, discuss the purpose of the theory.   Some points to address include:

Key Concepts:  As noted in 391, all theories include concepts.   List and defined the major concepts that are part of your theory.   This section can be a list of terms followed by a definition.   If you are quoting, make sure to use the proper citation protocol.

Empirical Basis:  Does the author include research (quantitatively or qualitatively based) upon which the theory is supported?   Can you find supporting research elsewhere?  How vast is the supporting research?   Discuss this in no more that 3 paragraphs.

Application:  Discuss and rate the theory is terms of its application to social work practice.   Does the theory provide a sound basis for conceptualizing social problems and/or human behavior?   Does the theory provide a sound guidance for your actions as a change agent?
