If you have previously earned graduate credits that will be more than 5 years old at the time your degree is awarded, it is sometimes possible to have them reinstated. Read this form very carefully; speak with your advisor; and, if any courses are appropriate for current degree requirements, submit the completed form to the Graduate School office for review by the Appeals Committee. Any appeal must be filed during your first semester of admission.
