• Application to Withdraw from Graduate School - (Use this form only if you are withdrawing from all courses for which you currently are registered and are not returning next semester.)

• Credit Reinstatement - If you have previously earned graduate credits that will be more than 5 years old at the time your degree is awarded, it is sometimes possible to have them reinstated. Read the two forms linked here very carefully; speak with your Program Director; and, if any courses are appropriate for current degree requirements, submit the two completed forms to the Graduate School office for review by the Graduate Appeals Committee. Any appeal must be filed during your first semester of admission.

Completed form, with program director’s signature, should be submitted to the School of Graduate Studies.  Please be aware that, if approved for 9 hours, you will be billed for university insurance for the semester unless you submit the appropriate waiver form to Student Accounts by their deadline.
