6. Send in the blue card received in the admissions packet
II. Request an intake packet specific to the disability.
III. Send in the intake form and the documentation specified on the documentation guidelines sheet.
IV. Wait to hear from the DSS office. A letter will be sent to you.
V. Make an appointment to meet with a member of DSS the first week of class by calling 910-521-6695.
