of $25.00 will be charged for returned checks. If you need to reschedule
your test date, you must call at least 24 hours before the test date
or forfeit your registration fee. |

Payment must
be made at time of registration. VISA and MasterCard payments are accepted during phone registration. You must register
at least one week in advance, however, since testing spaces often
fill before the deadline, it is best to register as soon as you are

| Retest Ticket: |
Repeat test-takers
must present the "retest admission ticket"
(RTT) mailed with the most recent personal MAT score report. If you
have taken the MAT since September 1991, you were issued an RTT with
the score report. This RTT must be presented at the time of
registration and again on the day of the test. If you do not have
one, you must contact The Psychological Corporation (MAT) at (1-800-622-3231)
and order a duplicate copy. |

| ID Required: |
To be admitted
for testing, each examinee must present two forms of identification:
one must be a government-issued PHOTO ID or a school
photo ID with signature and the second can be another form of identification,
such as a social security card or birth certificate. |

| Score Report: |
It is necessary
to know on the day of testing the name(s) of the institution(s)
receiving the score report and the name of the graduate program
involved. This information cannot be supplied later.
Score reports
are typically returned within 15 business days, or approximately
three weeks. Counseling and Testing does not receive a copy of your
score. The scores are mailed to the student and graduate program
only. |
